From Hospital to Home: Managing AKI: Ensuring Successful Post-Discharge Follow-Up - Interactive

Post-hospital management of acute kidney injury (AKI) focuses on ensuring recovery and preventing recurrence. This presentation will enhance knowledge and understanding of the key steps to successfully management post hospital AKI including: follow-up appointments, lab monitoring (serum creatinine, electrolytes, and urine output), medication review (adjusting or discontinuing nephrotoxic medications and optimizing doses of other medications), patient education (importance of hydration and nutrition along with signs and symptoms of kidney function decline), and preventative measures such as addressing underlying comorbid conditions, i.e. HTN and diabetes, as well as avoidance of nephrotoxic medications. By the end of the presentation, participants should be able to recognize, manage, and follow up with patients with AKI effectively, ensuring continuity of care and reducing the risk of complications.
