OTI: Module 7: Optimize the Diabetes Patient Journey: A Case-Based Approach for Busting Inertia

Therapeutic inertia is responsible for substantial, preventable complications in type 2 diabetes with associated excess in direct and indirect health care costs. During this practical training, we will take an in-depth look at a single patient journey over more than 20 years, exploring best practice approaches to optimize diabetes care, reduce complications, and minimize therapeutic inertia.


Learning Objectives

At the conclusion of this activity, participants should be able to:

  • Ability to implement a patient-centered diabetes care plan and an A1C-based follow-up algorithm
  • Understanding of which medications to use for post-prandial and fasting glucose improvement
  • Confidence in applying the ADA Standards of Medical Care in Diabetes to reduce therapeutic inertia in a patient with type 2 diabetes
  • Ability to describe the benefits of remote patient monitoring
