Serious Illness Management Toolkit Self-Assessment CME

This self-assessment CME activity aims to educate PAs more about the role of hospice and palliative care in caring for patients with life-threatening and serious illness. This activity is a collaboration between AAPA and PAs in Hospice and Palliative Medicine (PAHPM).

Learning Objectives

Upon conclusion of this activity, participants should be able to:
  • Define provider burnout
  • Define resilience
  • Identify individual skills and workplace factors which contribute to both burnout and resilience
  • Identify tools and resources that actually work
  • Define and discuss the role of primary palliative care for PAs providing care of hospitalized patients
  • Learn how to approach Advance Care Planning and difficult conversations in the hospital
  • Discuss best practices for managing symptoms at the end-of-life
  • Identify resources and options for care of patients at or near the end-of-life
  • Recognize the trajectories of end-of-life for different conditions
  • Identify tools and resources available to assist in giving patients a realistic prognosis
  • Assess which patients may be eligible for hospice services using the prognostic tools
  • Define advance care planning and its lingo
  • Discuss why advance care planning is necessary
  • Describe what might happen when you start the discussion of advance care planning with healthy patients
  • Create a plan to incorporate discussion of advance care planning with all patients
  • Define and differentiate palliative care and hospice
  • Discuss various delivery models for both palliative care and hospice and how to access and appropriately utilize each
  • Identify and explain the roles of members of palliative care and hospice interdisciplinary teams
  • Discuss and practice tools to approaching difficult conversations
  • Discuss and practice ways to partner with hospice to manage symptoms seen at the end-of-life; including pain, nausea, anxiety, delirium
